If going off just that image of the leaf, it looks like too much water. They prefer to be kept a bit dryer than too moist
Yeah you put them in the ground and then the flowers bloom on the tap root that comes out the end of the banana. They are then fertilised by earth worms and once the bananas ripen you then pull them out as the bunch we all know and love.
